Complete all 50 levels. Go straight for the blue gem to open the portal, then complete the level. Ignore the stars.
Even for the trophy, which requires you to get 3 stars, you can easily earn this at any boss fight. After you beat a boss, you will get the blue gem and the 3 stars in your path.
And there is one more trophy, for dying once, which will also come naturally.
